This notice covers Nakera Notice’s prelaunch website, its linked Emotional Load & Boundaries Check-In, and the separate hosted result form. These tools provide educational reflection and future practice updates, not counseling, intake, or emergency services.

ScoreApp check-in: information collected

The check-in opens on ScoreApp. The opening form asks for first name and email address. A separate, unchecked checkbox offers practice updates. Completing the check-in also provides answers to 20 reflection questions across five areas.

ScoreApp processes and stores submitted contact details, answers, calculated reflection results, and the opt-in status associated with your submission. Its account tools allow Nakera to review submitted answers and results. ScoreApp may also process technical information such as source and activity information to operate the check-in. This landing page does not receive or store your ScoreApp answers.

Please do not enter diagnoses, trauma history, detailed symptoms, medical information, or emergency information. Read ScoreApp’s privacy policy for its platform-level data practices.

Results and email delivery

The check-in provides an educational result on its result page after completion. ScoreApp can also send result emails when that feature is enabled; result-email delivery depends on the ScoreApp account’s settings. This landing page itself does not send result emails. Result-delivery messages are separate from optional practice updates.

Practice updates and your choices

Providing a name and email for the educational check-in does not require agreeing to promotional updates. The separate practice-update checkbox is optional and begins unchecked. If you choose it, your contact details and recorded consent may be used for future counseling-availability and scheduling updates.

You may unsubscribe from future practice updates using the unsubscribe option in an update or by replying to its sender. To ask about a submitted entry or request its deletion, contact Nakera through the sender of a practice update. ScoreApp provides account tools for removing or anonymising entries. Joining does not create a therapist-client relationship or reserve an appointment.

Separate hosted result form

If you use the separate ChatGPT Sites five-prompt check-in and result form, its questions are processed in your browser rather than saved to its database. The reflection theme is carried in the result-page link and may appear in browser history. That form stores first name, email, submission time, and any update consent. Counseling type and broad support-area preferences are collected only if you submit the optional follow-up after your result.

This separate form displays the result immediately and does not automatically email it or send practice updates. During the result-page session, you can join the update list or withdraw update consent.

Website hosting

The landing page and separate result form are hosted through ChatGPT Sites. Records submitted to the separate hosted form are stored in its Cloudflare-hosted database. These services process information to operate the pages and form. Private pages may require ChatGPT sign-in. No advertising trackers or additional analytics tools have been added to this landing-page build.
